Hello

I took my IELTS exam and got the results recently. However, the name printed on my IELTS report is GURJOT SINGH NA. I filled in NA , since there is no surname in my passport. But my current CIC application says the name as . GURJOT SINGH , since they had an option of leaving the "GIVEN NAME" column as blank, if there is no surname in passport.

Do I have to get an affidavit or attested document stating that GURJOT SINGH NA , printed on IELTS form and GURJOT SINGH are the same person?
