Queries about filling generic application form.....help plz

Hello,
can any one plz help me regarding these questions in the generic application:
1: Is FSWP a Family , economic , refugee or other ??
2: Is it a permanent holder class category?
3: Immigration office requested for processing?? I am applying from Pakistan so....
4: What is UCI in personal details?
5: I am presently single so my dependents are my parents or It doesn't apply to me??

I have some more queries can you please help me out for these as well:
1: Form for the additional dependent declaration doesn't apply to me??
2: In the background declaration form they are asking for my employment dates so I am working since FEB, 2012 till present son what should I write in:
From Feb 2012 till ?????
3: same is for my address since my childhood from (My date of birth) till -------???
