I am applying for outland sponsorship and completed my medical exam yesterday. I received a payment confirmation from the clinic and the receptionist said they would send the medical results directly to CIC. Does anyone know if this payment confirmation I have on hand is sufficient to enclose to my application as "proof of medical examination"? Thanks!
Were you given the 'Information sheet' with your name, DOB printed on it? That's what I submitted along with the reciept.
It says "Invoice" on the top of the page. It mentions my name (not my DOB though), date and that the invoice is for a medical examination for immigration with the amounts for the different test done. Also, they attached the receipt for the credit card payment.

ok this sounds different to what I got. I would say submit whatever proof you have that the medical was done, I'm sure the 'invoice' and credit card receipt will be fine.
When my husband had his medical done(in Canada) he received an Information page as well as the actual receipt. It says
E Medical at the top and has all his personal info (including passport info) as well as a photo that was taken at the Dr's office

That is what I was referring to - don't have it infront of me at the moment so couldn't recall the exact details. My medical was done in London, UK.
