I am a Naturalized Canadian Citizen and my child was in USA. We are looking into obtaining a proof of Canadian Citizenship for my kid -

As per the application guide on http://www.cic.gc.ca/english/information/applications/guides/CIT0001ETOC.asp , we are required to provide 2 Pieces of Personal Identification such as a Passport, a driver's licence and health insurance card, one of which much have a photo on it.

Now the issue is we have the child's USA passport but no drivers licence nor health insurance card at the moment. Are there any other known documents that can be provided as personal identification ?

1. Birth Certificate.

"Send copies of 2 of your ID's. I am sure they don't expect your son to have a DL yet"
 
Birth Certificate is already a requirement and I would guess it would count as an ID as well - Called CIC and they said, these circumstances are normal where a 4 month child is not expected to have a DL. The CIC person advised to just explain the situation in a letter and hand it in with the application, and provide as much identification as possible .

Hospital/ Immunization record is another valid ID. As per instructions....
