My husband just received his pre arrival letter. His Back check is still showing in process , When i called the 1888 number , i have enter all requred information and the automated service still say back ground check in process.
However I have sponsored him in May
He have also completed Biometric and Medical last month.
Can sooner please tell me what happen next.
Can they still refuse him after sending out the pre arrival?

The pre-arrival letter is often sent before all processing for the application has been completed. So yes, it's still possible to be refused after receiving the pre-arrival letter.
