I had applied for immigration in Sep. 2009.
1. On Oct. 2, 2012 I received email from London Visa office (LVO) to submit passport to LVO for VISA issuance.
2. I sent my passport for VISA issuance which was received by London VISA office on Oct. 9, 2012.
3. On Oct. 26, 2012 I received an email from LVO that the email sent on Oct. 2 was by error and your passport is being sent back without VISA as your file is still in process because of background checks.
4. Now my medical is going to expire in March 2013.
I guess this will be unique case. Anyone can please comment that what will be the expected reason of all this?

It sounds like the PPR was sent to you prematurely. So you might get your passport back only to have them send you another PPR.
What this means is that whoever reviewed your file at the beginning of October made a mistake, because your security checks ("background checks") were not really done. So when the officer who issues the COPR reviewed your file, she or he noticed the error and send you that second e-mail.
The only question is how long your background checks are going to take. Since that isn't even done by CIC - it's done by CJIS - it may be done in a few more weeks, or it might not be until next year.
That really is an unfortunate error, but it's not a fatal one.

Just a mistake. If there were something wrong with your case, the officer would be communicating that fact to you. There are strict rules around "problems" and the procedural rules they must follow.
