from any government hospital. Do remember to take your passport with because they will issue a polio vaccination card in which they will write down your Passport Number
 
But i have already sent my original passport to the canadian embassy. I will take the photocopies

yes take the photocpies, and also keep ur original CNIC to be on safe side
 
I got appointment on 27th novemeber. I have applied for the january session. Masters program. Do you think i will got the final results of visa before my clases? My letter of admission expires on 19th january
