I am basically from India and currently in Canada.

1. Do we require Police verification from all the states that we stayed in India?
2. I am working in Canada from last 1 year. Do I required Police verification from Canada as well?

Only one PCC is issued irrespective of where you stayed in India. So it is one PCC per country.

Not sure general processing time
But one of my friend applied it in India and he got in 1 day.
If your are applying from Canada in BLS office, you will generally get in 10 working days.
