Police clearance certificate - SFO Embassy

I am applying for a PCC with SFO. Their questions are little ambiguous and I AM LOST.

8 a) permanent address & tel no along with police station

What does this mean ? I put down my address from my passport and my landline from India. What do I have to do for the police station part?

8 b) permanent residential address and tel no, along with police station and residing since

I put my current address for this, but I have not lived in this address for a year.

9) if you have not been resident at the address at column 8 b) continuously for the last one year, please furnish other address with duration resided ( please furnish an additional set of pp forms for each address with police station.

So I just print out another pp form and just fill out from and to for question 9 and staple it to my original form ? Or refill the entire paper? And what to do about providing address with police station ?

These three questions hurt my brain, I was planning to apply via mail. This makes me want to directly go to the embassy as the questions are really confusing. Any pointers are appreciated.
