Police check/certificate

About to order police checks from Sweden (home country) and Australia (working holiday there for 2 years before coming to Canada). The doc checklist says to prove " A: Background/Declaration (IMM 5669) form for yourself (and any family members 18 or over), and b) provide police certificates."

As im looking at it (aus one at least) theres a polis "check" ("lists individuals criminal and traffic court outcomes and pending charges" and police certificate "official document issued as a result of a background check by the police or government to enumerate any criminal records the person might have"

Any idea of if I order both or is just the check needed...?
