Hello, is taking my husband's lastname considered to be a "legal name change"? I have put in point number 5 my name at birth and my married name (this is the name I use now), should I be providing any supporting docs to prove it was legal? all my IDs, PR card and passport are already with my new name. I have checked our Canadian Marriage Certificate, it has my maiden lastname only.
Or mybe should I leave out the married name from the section number 5 and keep just my name at birth since married name goes to the main application form. will I need then provide marriage certificate as well?
