I have short listed few forms and would like to share with you all for input/ suggestions and mentors to each other.Having said this I have shot listed below forms/documents from the checklist form IMM 5690:
I know this may vary from person to person, but believe these are bit universal and key documents that might apply to majority of us. Please provide your comments if I am on right track.

I don't think so we need to fill in the education details except in brief in form IMM5669E under section 7. Despite this I couldn't find any other forms in the list below with reference to education, I believe we have already gone through this phase in ONT pilot so no point now.

UCI means Universal Client Identification.
It's a unique ID number which CIC assigns to every applicant in order to track your application/immigration history etc. If this is your first time dealing with CIC they usually send your the UCI with your AOR.
But please confirm the versions first from the bottom of its each page in fine prints and ensure this should be same as with CIC's official site.
Sorry I cannot help you about your payment options in Pakistan, but I am sure some one from Pakistan might help you on this if getting through this forum/ thread....but shouldn't be hard though. You can do some google search of the payment methods used in Pakistan, am sure you will get your answer.
Alternatively make sure you also got updated version of adobe PDF reader or any other PDF reader.
You have to download adobe 8.1 or adobe acrobat 8.1.
I'm facing the same confusion regarding fee submission, if I submit in PKR as per CH ISB or pay in dollars. Let me know regarding fee submission if you find any relevant answer.
I believe it should be in Canadian dollars. You can make a bank draft.
