Can tell me that am I eligible for Canada Fast Track FSW program?

Name: Sami
Education: BSc (14 years education)
Additional Edu One year Diploma in Information Technology DIT
Total Education 14+1 = 15 years.
Worked Experience: More than four years (From FEb 2005 till current) as Manager Network Design and Development.
Adabtibility points 5, as my Uncle lived in canada.
Language Got 6.5 bands in IELTS
Age 26 years old


Please suggest that may i apply for fast track program?
