I am the principal applicant. My wife was rerefused for an American tourist visa in 2013. I forgot to put this information in the form of IMM5669. Because I am a Chinese applicant, my case will be sent to Hongkong. Can I take the initiative to fill in the missing information to HONGKONG CIC branch?

You need to correct this error asap - it can result in a misrepresentation refusal and a five year ban from Canada. You should contact your visa office as soon as possible to notify them of this error and then submit a corrected form with the refusal listed.

What if the US visa refusal occured after submitting imm 5669 in like 6 months, is it considered misrepresentation and is it needed to update cic regarding the refusal?

In the form of IMM5669, there is a question like that " e) been refused admission to, or ordered to leave,Canada or any other country? yes or no"
My question is " refused a US. Visa" means "refused admission to US " or not ? Is this the same mean? I have been refused a US visa in 2013. Should I choose yes or no?
