If space is not sufficient we cana attach a word document right.
Moreover do we need to sign the word document too or only name of applicant and name of form is fine

hi ganpathi.. i created extra sheet and marked it as EXT_Schedule A_Background info: and created tables similar to that in the form and added the details making the points correctly. I did not sign the extra sheet.

I signed additional sheets whenever the original sheet required a signature.
