Photographs for Medical Exams

We're about to get the medical exam for my partner. One of the doctor's I contacted said we needed to bring 3 passport photos and the 'peach form'. I assume the form is just the print-out of Appendix C, but I can't find any information about the required photographs. I remember reading that 5 pictures were required for the medical exam, but i can't find where I read that - I think it may have been in an old instruction package (pdf format). There were no specifics other than that they had to have been taken within the last 6 months. But I can't find any instructions in the current online guides.

I'm just wondering what is really needed in terms of photographs for the medical exam.

the number of photos is up to the doctor. the one we are going to see asked for 4 recent passport photos. I have heard of some asking for 3, 4 or 5. it depends on the doctor. we also had the date stamped on the back when we took them.

and yes, you take the form from appendix c, they keep that then provide you with another form that you send in with your application

for me the doctor ask 4 passport photos and appendix c and after i done hi give me some other form i send to my wife than she send with the application
