Photograph for Permanent Resident.

I need to upload my photograph as the final step of the Permanent Residence confirmation process on the PR Confirmation Portal.

I am not sure which type of photograph I should upload. I got my photos (both digital and paper) from a professional photographer.

1) Should I upload the digital copy (.JEPG), or should I scan the paper copy?
2) Should I upload both sides of the photo? The backside of the photo has the studio's name and the date of the photo taken.
3) Should I write my name and DOB backside of the photo with the maker?

IF anyone can guide me, that would be helpful.
