I am quite confused with the photo requirements on which of the following is correct (6 photos in total for sure):
a) 5 dated with name and DOB, and 1 dated WITHOUT name and DOB;
b) 5 dated with name and DOB, and 1 totally blank.
The instructions on the Document Checklist #14 and the online Appendix A are different. Checklist says "...leave the sixth photograph blank." but the Appendix says "...bear the date the photo was taken..."
^ Me and my family had our photos taken at Walmart. I asked the photographer to leave the back of 1pc blank and the other 5 with the date the photo was taken. When I got the photos, just as I requested, the back side of 1pc is totally blank, the other 5 has a stamp with the date, place the photo was taken and the initials of the photographer(I think). Then I just wrote my name and date of birth on the blank spaces left. The stamp filled the whole back of the photo.
