Country specific requirements
Philippines - Additional forms for residents
  • There are no extra forms for this country.
Philippines – Instruction(s) for documents
  • The following applies to civil documents issued in the Philippines:
    • Birth certificate issued in the Philippines: Provide the original birth certificate issued by the Philippines Statistics Authority (PSA) and printed on PSA paper. The principal applicant and all family members included in the application must submit this
    • Marriage certificate issued in the Philippines: Provide the original marriage certificate issued by the Philippines Statistics Authority (PSA) and printed on PSA paper.
so it means that we need to submit the original copy of psa birth certificate and marriage cert right?

ALSO, this confused me too... should i also submit psa advisory on marriages?

If you hold a Philippine passport, you and your family members included in the application must follow these instructions:
  • Principal applicant: To establish marital status, the principal applicant must submit either a Certificate of no record of marriage (CENOMAR) to establish that there is no record of marriage, or a Philippine Statistics Authority (PSA) Advisory on Marriages providing details about any marriages registered in the Philippines.
i hope someone will answer this question.. thank you :)

Are you sponsoring ur hubby
no, my husband is sponsoring me but i'm the one organizing the papers :) so should i still need to submit psa advisory on marriage? thank you :)

Marriage certi... is a proof you are already married but if you can provide the cenar as well it wont affect the processing.

thank you so much :)

No problem
confused again, it says on the checklist page 5 IDENTITY DOCS, TRAVEL DOCS AND PASPORTS "You (the prncipal applicant) must provide the following PHOTOCOPIES only" T.T btw my husband will be the one who will send the application in canada

For passport you will send photocopies the original they will request it later on once your visa is ready.

We didn't send a PSA CENOMAR, only a PSA marriage certificate and we have had no problems so far. It wouldn't harm to include it though.
