Do I need to send a form called "Personal Net Worth Statement" to CIC in Buffalo, NY or they do not require it anymore?

Yes. For pre Feb-2008 -Applicants.

For post-feb2008, I don't know. Maybe Rupesh and others will be able to answer this part.
