Permission Release and Consent Form - Minor

Hi,


I have a question. Our Oath ceremony (for my self and my 6 year old son) is on 15-Jun-2018. I am not bringing my 6 year old minor son with me for the Oath ceremony. I have prepared all the documents and while filling the "Permission Release and Consent" Form, I am confused. In my Permission Release and Consent Form in the field of Minor Children Print Name and Given Name I will give my 6 year old son's name. But I am confused that for my son's Permission Release and Consent Form do I need to fill it or do I need to leave it blank as I already given my son's Name in the "Permission Release and Consent Form" of mine
