Permanent residency (no express entry) timeline:

• Date Received: December 13, 2019
• Date of Biometrics Enrolment: February 26, 2020
• Pre-arrival letter: July 17, 2020

Does anyone know which is supposed to be the next step? And how long usually takes to receive the medical test request?

Most of us who applied before July 2019 got Medical req before pre arrival. But in your case its a last step. So expect it with in next few weeks. And according to me if you application AR date was Dec 2019 then you should expect pr before Jan or Feb 2021
