PCC submission post ITA profile submission

Hello everyone,

I submitted my ITA profile last month and now, an officer has asked me to upload my PCC as it was delayed at the time of my ITA profile submission.
After I hit the upload document button, the following questions appear before I get to upload the document.

1.) Document name
2.) Document number
3.) Issue date
4.) Expiry date

I need help with the following aspects.

A.) What to mention in 1, 2 and 4.
B.) Is the document number same as the file number mentioned on PCC?
