Passport submission- VFS Bangalore

Hello all,

I need information on the documents we need to include for passport submission to Vfs Bangalore for PGP sponsorship.

The website mentions about IRCC Appendix A form to be included with other doc. Is this correct?

If someone has already submitted their parents passport to Bangalore vfs please provide the list of docs you sent.

Don't know Bangladesh, but one document known as Appendix A - not generally available but referred to by IRCC/VFS - is just a basic form listing each applicant + dependent included in the app and providing height and eye colour.

so include tombstone data (name, dob, passport, UCI) and height and eye colour in a simple table, any basic table will do (eg in word or excel).
