Passport Request - What Documents To Send?

Hello Everyone,

We received PPR (Passport request letter) and they ask to send a passport, a copy of this letter, and two photos.

However, I heard that they usually request Appendix with the information like eye color and height. They did not request anything from us.

Should we still attach a piece of paper with this data?
