passport request form!!!!

Hello there!!

I just want to know that is it standard passport request form (P152E PHEP Request.pdf) for every applicant. CIC send me an email for passport but they probably forgot to attach the above form with it. Could i wait for them to send me the form or should i borrow this from my friend and send it to them. please share your views.

I dont know what this form is.I have not heard of this when they ask for you to send in the passport you do just that.I dont think i ever heard of anyone having to fill in a form with it.But i could be wrong.I tried everywhere to find this form and to no luck.

on our passport request form in the bottom - I believe it's the form name - says (02/10/2008) -50531
Don't use friend's form... just ask them to resend the letter. Mark your email as high priority(if you are using outlook or windows mail)
We just included the copy of this letter with the passport sent to CIC

in fact its a instruction and information pack with an updated information chart in it which needs to be filled by a main applicant and get it submitted along with passport and 2 photographs. i meant that updated information chart in my last thread.

how can i be sure that the passport that ive send to the embassy of canada,Ph..was already received..
I've send it tru LBC...
..AND IF THEY ALREADY received it..how long will i wait for the visa?
