My mother had a first name change(Old Name i.e. Pooja Mehta) after her marriage(New Name i.e. Divya Joshi). Now, all my mother's document has her new name. I am filing for my mother's visitor visa in Canada and I wish to declare that she had a different name before marriage. I am preparing an affidavit to state this fact and notarize it.

The above affidavit format is for only surname change. I am looking for a format for first name change as well. If someone has any recommendation or a format to recommend, I will really appreciate your help.
