Open Work Permit (PGWP) Extension

Hello,

I am a holder of a PGWP that is an open work permit (i.e. I do not have a certain employer listed there).
I am currently applying for PR as I have received an invitation to apply.
My work permit expires within 4 months, in late June, and I do not know how to answer one of the questions in the form for an extension of my work permit.
I know that I am eligible for an 18-month extension, but a 12-month would do as well. I am having difficulty with this one question.
The form is IMM 5710E and it says:

"Check the appropriate box or boxes to indicate if you are applying for:

• A work permit with the same employer
• An initial work permit or a work permit with a new employer
• Restoration of your status as a worker
• An initial or an extension of a temporary resident permit"

Which option is the right one for me? This form seems to be the same for a bridging work permit.

P.S. How long would it take me to receive a completeness check and AoR from the moment of application submission, so I can start my Bridging Work Permit application?
