So while filling out the Nominee Application form, according to the instructions all the questions that aren't applicable should be marked with a 'N/A'. Obviously for Dates and as such this isn't possible. But I was wondering if the N/A has to be marked by hand after the form has been printed out or the spaces can this be left blank ?

Yes, you can write all N/A's after printing out. That should be fine.
