ontario legal change name , help please

Hello,
I was going to legally change my first name on the ontario website, but I have a few questions :

- first my permanent resident card is expired and the application requires a photocopy of it, is it ok to apply with an expired PR card or does it have to be renewed first ?

-The application says that you must have lived in ontario for at least the past 12 months, I have had a residential adress in ontario for 5 years, however during the last 12 months I was overseas for 3 months for vacations, is it ok as long as my residential adress was still in ontario or do I have to be physically present in ontario for 12 months ?

-Can the guarantor be the manager of my bank ? (scotiabank)

This seems unrelated to immigration, you might be better off contacting Service Ontario directly, using a telephone and talking to a live person, if you don't feel like visiting an office.
