Obtaining medical forms- outland through Buffalo

Hi all,

I'm new here, and hoping to submit all my documents in May. I'm scheduled to do my medical exam 5/10, and confused about what documents I'm supposed to bring with me.

I have Appendix C for USA applicants, but am I also supposed to have IMM 1017 EFC and/or IMM 5419? If so, where do I get those forms? The doctor's office only told me to bring "immigration forms" with me, and I want to be sure I have everything I need.

Hi, if you are applying outland, you need your Appendix C and photos. The doctor will give you the Section A paper which you include in your paperwork.
