I think the 3rd GU that you're talking about is not actually a GU. It's the date your tracker was updated to reflect the BG Completion
I've the same dates as you and my tracker was updated on 31st to reflect background check that you see completed on 27th
I used larger paper clips for each set and put a post it naming each set . e.g. Photo identification proof: Passport biographical page and health card. then i put a cover page before each applicant's name with the index of all the document included. so they know that next person's pages to follow
Mine is family application and I am keeping everyone application in separate envelope (All papers are loose) and then all envelopes in single large envelope. I hope nothing would wrong with it. Please correct me.

Hi everyone, there’s an important notice on the test invitation asking to provide scanned copies ofany passports or travel documents I held thru the eligibility period.. I have my original passport and it’s expired but don’t got the travel document cuz I lost it while ago and it was expired as well. I never made a police report. Does everyone get this same notice ?

Yes, online applicants generally receive this request. Some times paper applicants do too. Make sure you attach:
1. your all pages of expired passport +
2. any copies of your lost/stolen travel document, if you have them (hopefully you have copy of all pages) +
3. police report for the lost/stolen travel document
4. list of travel dates on your travel document and proof (e.g. tickets)
4. cover letter explaining the loss/theft of your travel document and you are attaching the above documents
