Photocopy of two (2) pieces of personal identification.
Both pieces of identification should show the minor’s name and date of birth, one of which must have a photo on it.
Examples include:
permanent resident card (PR card)
A PR card may only be used as personal identification if your second piece of personal identification was issued by the Canadian government
school record;
health insurance card;
a passport page containing their photo and personal details;
a hospital or immunization record, may be provided.

Sorry, I am not answering your question, maybe the expert members can advise about that, Can you please advise how long it takes to get this response from them? Did they mail the full application back or did they send you an email?
I have submitted my application at the end of November and waiting on AOR, regarding the (2) pieces of personal identification for my kids, I submitted PR copy + OHIP Card+ landing paper. I hope it's acceptable as per the above guide.

I'm not an expert but maybe a two months old signature is an issue, new dates will require changing the presence test and the app. Again, I'm not an expert

I think it is better to submit the application with new dates and signature. If you are worried, you could include a letter stating that you have reprinted the same application with new dates and signed it.

Hi, I think for any application online or paper, if you should submit it again you need a new physically present form and based on the new form of course everything (dates on the different sections) will change.
