Notice of seizure form

i am schedule for pr interview and the letter says to bring the certified seized copies which i knw but i am not sure of the Notice of Seizure Form can sm1 plzz help me where can i get it from

Hi
When your original IDs was seized by CBSA they gave you a true copy stamped stapled with a form. That's what they want you to bring.
