The example you gave is considered 2 words.
They didn't say you need for use good grammar - just write 3 words or less that suits the situation.
For eg., if a question's right answer is Good, Bad and Ugly. Since it has four words including "and". In this situation , you have to write as 'Good+Bad+Ugly'. They will accept this answer.

Actually April 21st, 2014 is correct - 4/21/2014 is incorrect
If you use
only numbers, then it
must be date/month/year - this is done to avoid confusion between Americans and British styles: American style = month/date/year - British style = date/month/year
If you write out the month in English, there are 3 possible answers:

21st April 2014
April 21st, 2014
If you don't write out the month, there is only one way:
21/4/2014 or 21/04/2014

April twenty first two thousand and fourteen. if written is more than 3 words, so it can't be written in 3 words.

Both ways are fine - up to you man. if you don't want to write out the month, then don't write it and just use numbers. this is just one small part of the four sections in your test - don't over think it and make it more difficult than it really is.
It's the most complicated question because you decided it's the most complicated question. it's really straightforward and you can go to ielts official site or call ielts organization and ask them if you're not sure
