There is document check list for this, which is specific to visa office; first you must use that as cover letter (first document of your application).
You have made specific changes to you your case; so how can anyone else cover letter can help you? Please write a letter (addressed to your) visa office and mentioned what changes you have made. you can make a table having header column like (Form no. - point no. - mentioned in application to CIO - Updated text (application to VO) - Reason )
