Need help with student permit application.

My student visa got rejected because I didn't provide the English proficiency test results. I'm reapplying and I need to know where exactly I can upload the document. As such, they're asking for the family information form passport photo, offer letter bank statement and work requirement in study. In addition, there's the optional client information and the application for temporary resident visa. Please help! Thanks.
