While filling IMM 5569 using computer,Do I need to type name in sigature box and select date .As i m mailing these forms...do we need to sign by hand as well? If yes,In the same signature box where you type the name.

Schedule A – Background/Declaration (IMM 5669)

You must do the following when submitting a paper version of the form with your initial application:

• In the signature section, type the name of the person for whom the form is being filled out. Also enter the date that the form is being filled out.
• Validate the form.
• Print the form.
• Sign your name by hand. (Note: For principal applicants under the age of 18, a parent or legal guardian must sign the form on the dependent child’s behalf.)

Important: If the form is not signed and dated per these instructions, it will not be accepted and your applications will be returned to you without being processed.
