need help to find a form for travel document for permanent resident

Hello.

I need some help and advice please. Where do I find the application form for the travel document for permanent resident.

Scenario:

My husband landed as the PR on Feb 14, 2008.
He needs to go back to England to start a tour in May 15, 2008.
The PR card has not arrived yet.
All we have is a record of his landing and confirmation of PR (which is important document)

What I have been told by the immigration people is that I need to find an application form for the travel document for PR.

Does anyone know where it is on the website? I understand that I've got to do this in England? Can't be done while in Canada? Is that right?

any help and advice is appreciated.
