Need Help regarding Cover Letter

Hi Guys,

Since my employer has refused to give me the letter of reference stating my roles and responsibilities, working hours, etc on company's letterhead, I am going ahead with the alternative way of getting an email (through company's official mail ID) from my supervisor and attaching affidavit and his ID card copy.

I am thinking of writing a cover letter to CIC to help them understand my situation and why I was not able to produce the required Letter of Reference.
Could someone please help me in sharing some sample cover letter.
