Need help in filling the "Work/Education" section for Maternity Applicant

Hello All,

Please help me in filling the work/education section. Planning to send my application on 05/10/2017. I am already eligible from 04/15/2017

My case:

1. Landed in canada on 03/29/2013
2. Started Full time work for xyz from 05/2013 to 04/2016
3. Went on Maternity leave from 05/2016 to Present
4. I will be back to work from 06/2017


Please help me how to enter the above in the work/education section as below:


FROM TO WORK/EDUCATION EMPLOYER CITY/COUNTRY

FROM TO WORK/EDUCATION EMPLOYER CITY/COUNTRY
2013-03 2013-05 write exact dates of unemployment reason for unemployment city/county you were present/living that time
2013-05 2016-04 WORK (you may write your designation) name of employer city/country you were working
2016-04 present exact dates of maternity leave on maternity leave city/country you were living since 2016-04
