Need advise!

I landed 08 and never worked and stayed in canada for two yrs. I have been working in the u.s since I landed but last yr November 13 I got a job offered from canada so I quit my job in the u.s and moved back to canada and started my new job in December. The question is when I file my t4 for December 2013, do I have to tell them that I worked in the u.s? I only worked a month in 2013. For the last few yrs I only traveled back to canada twice or three times a yr. every time I stay for a wk or two. I really need help on doing my tax. Thank you!
