Need advise on OWP renewal

Hi everyone!
I need ur help on my OWP. I just got it last dec 9,2011 and it will soon gonna expire. (dec. 2012)
I want to renew it b4 it expires but i dont know if what form should
I use and if it is ok to renew my OWP without an employment contract? my employment contract will also expire on dec 2012 but i dont want to renew it anymore
I want to look for a new job since i got my OWP.

My questions are;

should i still use the same form
as i applied for an OWP?

Is it find to renew my OWP without an employment contract?
If not, then can i use my old employment contract since it is still valid til dec 2012?
