My wife retained her maiden surname.. and did not change her name on any document officially. But before deciding that, she changed her name on Facebook and included my first name as her surname on Facebook on the day we got married. Is it an issue?

Part 2 of Sponsor's personal details asks "Have you ever used any other name" (Nickname, maiden name, alias). Should the sponsor mention this name that she uses only on Facebook?

Facebook name change isn't important. If your wife has used any other names before married or she chose to officially used your surname, then she must declare it.
It is not necessary to mention the Facebook name. However, it is OK to do so, and if she is going to use any Facebook posts as part of your evidence, I would just include the name in the answer to this question, saying it is the name she uses on Facebook.
