I am currently applying for a study permit as an international student. My name is Vo Trung Son. Vo (family name), Trung (middle name), Son (given name).
On my OUAC form, I filled in my name with Trung as my middle name and Son as my given name and it was formatted as Son Trung Vo. This is the name on my university's offer letter.
However, on my passport, my middle name and given name were grouped as Trung Son, therefore, my full name on my passport is Trung Son Vo.
Will Son Trung Vo and Trung Son Vo on my offer letter and passport respectively be a problem on my visa application? Should I tell the school to change the format of my name? Thank you in advance.
It's best to make sure all your documents have the exact same name as your passport. You don't want to run into any issues later on.
Ask the school to change the name on the letter so that it matches your passport. Send them a copy of your passport's bio page so that they know exactly what it looks like.

Why on earth would your school be cranky about putting the correct name on your admissions letter? That's the name that's going to be on all your transcripts, your diploma.. do you really want to have this mistake perpetuate itself?
When it comes to immigrating to/studying in a new country, you want to make sure that ALL documents have the same basic information (Name, D.O.B, etc). If you have discrepancies, it's a huge pain trying to correct things later.

If I were you, I would put Given Name as "Trung Son" and Family Name as "Vo". Leave the middle name field blank. That is what I did. I follow the same rule for all other documents so that my name appears consistently.
