Hi my situation is as follows- I am 30 yrs old and I got my citizenship card when I was 14 however the middle name on it is my nickname rather than what my actual middle name is which is on my birth certificate. Since the citizenship card had my nickname on it my health card ended up being the same too. My question is how can I correct my middle name on my citizenship card and what is the fee and process time it takes?
Also on a side note does anyone know if the name that is on your SIN card is the same that would have been on the application form OR does the form allow you to put your full name on it but then on the actual card they just print your first and last? Thank you- (the reason I ask is the same as my first question)
Apply for a new Citizenship card by following the instructions at http://www.cic.gc.ca/english/citizenship/proof-how.asp#step1 The fee is $75 and processing time 10 months although you can ask for it to be expedited if you need it sooner.
Once you have that corrected, you can use the new citizenship card to correct your health card. For your SIN card, you need to ask Service Canada if they can have your full name in there and only print first and last.
