Hi - All my educational certificates having only my FirstName(example - ABC) and I got my name changed to FirstName Lastname (Ex- ABC XYZ) while applying for passport for Work related travel.Please let me know what should be the response to below question ?

Has this person ever used any other name ?

If I answer as Yes to above question it shows section to enter previous names where , last name is mandatory. Should I enter as FirstName in both FirstName and LastName or should I enter FirstName alone in LastName section ?
