Hi all,
I have changed my first and last name in India. My all id proofs and passport have my new name but, my birth certificate has previous name. I do not have original name change document. Can I provide affidavit which declare that I am the same individual as stated in the birth certificate.
Any help would be appreciated.

You wont need your birth certificate anywhere for express entry application. Just make sure all other documemts like IELTS TRF, education documemts have name same as passport. If there is a mismatch there, then you will need to explain your situation in letter of explanation. Also, upload documents used for your name change along with LOE if this is the case.
All my Canadian education documents and id proofs have name according to passport.
