I am I will get some hate on this but any help would be helpful.
I live in Canada and my wife is from Pakistan. We got married in December 2017. Before our marriage, we are also cousins (i know many religions and people are against it). We have never met before our marriage so I think it is fine. I am applying for spousal sponsorship for her but I got stuck on one question in form IMM 5532 part B) Information about the principal applicant.
The very first question they ask, is the principal applicant related to you in any way before marriage? and then asked for people my wife is related to in my family?
I don't know what names should I put, should I list all family member's name like, my father, mother and siblings?

You need to answer the question honestly. If she is your cousin, you wouldn't list your entire family, you would just list the family members she is related to by blood (so for example, if your mother is your wife's aunt because your wife is the daughter of your mother's brother, you would list your mother and write she is your wife's aunt).
You do not have anything to worry about, there is no immigration policy that blocks the sponsorship of cousins who get married. The only limitations (and reason they ask this question) is that marriage/sponsorship within immediate family (grandparents, parents, siblings) is not allowed.
Moreover, there are couple of questions relating how we met and asking for dates we met before.

No, of course you shouldn't make up a story. Lying in the application is immigration fraud. You just said in your first post that you never met before getting married.
