In the medical process, I see a form needs to be submitted with Medical History where first question asks about TB history and Treatment details. The options are Yes/No and if Yes is chosen further details need to be provided related to treatment.
I had a major infection during early childhood (over 30 years back) which my parents do no recollect whether was confirmed TB or not but I had a three months long treatment for the same. We do not have any diagnosis or Treatment history since the history is very old. I am thinking what option should I chose in Medical form.
Option 1 - Chose Yes in the form and mention No Diagnosis/treatment details available. The risk is if it was not TB then also may create considerations for further medical tests and delay the process.
Option 2 - Chose No and mention 'Do not know. Had BCG vaccination in childhood'. I assume, the form will allow text if we chose Option 2.
Question is if we chose Option 2 (with/without comments) and further if it is revealed that there is old/inactive TB, can that be considered as misrepresentation whereas we truly do not remember the history ?

A three-months' treatment does seem like a treatment for TB. To be doubly sure, you can do similar x-rays/tests at a private clinic. TB may still show up as inactive TB in these tests although you were treated in your childhood.
Not declaring it would be misrepresentation like canuck_in_uk has pointed out

I had an x ray done earlier and a pulmonary nodule was found, but doctors could not infer from that whether it was a result of a TB for sure, as it could have been a result of any infection like TB, pneumonia. Since the history is so backdated and I really do not have any reference, medication details, reports etc, I find myself in an awkward position. At the one hand declaring Yes without firm knowledge on history of TB and without any relevant details is a concern and on the other hand Declaring No is also not a confident answer. Unfortunately we do not have a Do not Know option.
Did anyone have similar situation (50-50) chance of TB history and no knowledge about it ?
I was talking about the medical form IMM5419 . Does anyone know if it has to be submitted in APR application or at Doctor's office as a hard printed copy ?

Makes sense. Thank you for your suggestion. I shall follow the same.
If Doctor submits the form, shall I be given the form to fill up the section or Doctor will in my behalf ? I know it may vary from practice to practice, but in case you have a general idea.
